6th Grade Immunizations

Dear Parent(s)/Guardian(s) of 5th Grade Students:

The New Jersey State Health Department has established immunization requirements for all students entering or transferring into a New Jersey school who will be going into sixth grade in September 2021. In accordance with N.J.A.C. 8:57-4, all students entering sixth grade on or after September 1, 2021 must receive the following immunizations:

  • One dose of Tdap (Tetanus, Diphtheria, Pertussis) vaccine; AND
  • One dose of Meningococcal vaccine

This means that if your child is entering sixth grade in September 2021, you must provide documentation that your child has received these vaccines. If your child has already received the two vaccines, please provide the school with documentation from your physician before the end of this school year.

If your child will be receiving the vaccines during the summer months, you must bring the documentation to the middle school no later than September 1, 2021. The middle school will be open during the summer months for you to bring the necessary documentation. Additionally, if you are unable to make an appointment with your physician prior to September 1st, you must inform me of the date of your anticipated appointment.

Please note, your child will be excluded from school without the proper documentation of the completed immunizations or notification of the anticipated appointment.
