6th Grade Mandatory Immunizations


Dear Parent(s)/Guardian(s):


The New Jersey State Health Department has established immunization requirements for all students entering or transferring into a New Jersey School who will be going into sixth grade in September 2023. In accordance with N.J. A.C. 8:57-4, all students entering 6th grade on or after September 1, 2023 must receive the following immunizations:

  • Tetanus, Diphtheria, Pertussis (Tdap) – All incoming 6th grade students are required to have a booster of the Tdap vaccine
  • Meningococcal (MCV4) – All incoming 6th grade students who have had their 11th birthday must have one does of the MCV4 vaccine


There will be no grace period for students who have reached the age of 11 by August 28th, 2023. Students must have documentation of these vaccinations submitted to the school nurse prior to the first day of school. If the student does not turn 11 until after the first day of school, he/she will need to meet these requirements within 14 days from his/her eleventh birthday. Please note, the main office is open during the summer months and the information will be given to the school nurse.

In the event immunizations are not received, a note from your medical provider’s office of a scheduled appointment for the above mentioned immunizations will be required with follow up documentation in order to attend school. Student records are inspected annually by the Health Department for compliance.

Please note, your child will be excluded from school without the proper documentation of the completed immunizations or notification of the anticipated appointment.
